DSSW’s programs are designed to assist persons with disabilities in achieving the greatest degree of independence possible within their community.

The evaluation process by a nurse and the interview process by a program director and your independent case manager assure that an individual’s needs are identified so that they can be met by the services available in each program. CLASS (Community Living Assistance and Support Services)

This is a Medicaid waiver program offered by DSSW. Services under this program include:

PHC (Primary Home Care)

This is a Medicaid waiver program offered by DSSW. Services under this program include:

  • Escort: 
    • Accompanying clients on trips to obtain medical diagnosis, treatment, or both. Does not include the direct transportation of the client by the attendant.
  • Home Management:
    • Changing Bed Lines
    • Housekeeping
    • Laundering
    • Shopping
    • Storing Purchased Items
    • Washing Dishes
  • Personal Care Assistance:
    • Bathing
    • Dressing
    • Grooming
    • Routine Hair and Skin Care
    • Preparing Meals
    • Feeding
    • Exercising
    • Self-administered Medication
    • Toileting
    • Transferring/Ambulating

CDS (Consumer Directed Services)

This service delivery option allows recipients of eligible programs to manage the delivery of a range of services including attendant care. CDS is administered by DSSW’s corporate office. This delivery option gives participants more flexibility in their schedules, greater freedom to select attendants and to structure their pay.

Eligible Programs Include:

  • CLASS (Community Living Assistance and Support Services)
  • DBMD (Deaf-Blind-Multiple Disability Waiver)
  • MDCP (Medically Dependent Children Program)
  • PHC (Primary Home Care)
  • CBA (Community Based Alternatives)
  • PCS (Personal Care Services)
